Peyronie hastalığının cerrahi tedavisinde kullanılan greft türleri

Peyronie hastalığı penisin tunika albuginea tabakasında fibrozis ile seyreden, nedeni tam olarak bilinmeyen ve kalsifiye plak oluşumuyla sonuçlanan bir patolojidir. Bu plaklar peniste ağrı, eğrilik, kısalma ve daralma gibi şikâyetlere neden olabilir. Klinik tablonun stabil hale geldiği ciddi durumlarda cerrahi tedavi altın standart yöntemdir. Hasta kliniğine göre üç farklı cerrahi teknik uygulanabilir ve greft kullanımı ile penil tunikal uzatma işlemleri bunlardan biridir. Penil tunikal uzatma cerrahileri için otogreft, allogreft, ksenogreft ve sentetik greft olmak üzere dört farklı greft tipi kullanılır. Her greftin kendine özgü avantajları ve dezavantajları mevcuttur. Bu derlemenin amacı Peyroni hastalığının cerrahi tedavisinde kullanılan greftlerin avantaj ve dezavantajlarını güncel literatür eşliğinde sunmaktır.

Grafts for surgical treatment of Peyronie’s disease

Peyronie’s disease is a pathology of uncertain etiology progressing with fibrosis in the tunica albuginea layer of the penis and resulting in calcified plaque formation. These plaques may cause pain and deformities such as penile curvature, shortening, and narrowing. Surgical treatment is the gold standard in severe states in which the clinical course of Peyronie’s disease has stabilized. Three different surgical techniques are applied, depending on the patient’s clinical manifestation, one of these being graft use and penile tunical lengthening procedure. Four types of graft are employed for penile tunical lengthening procedures –autografts, allografts, xenografts, and synthetic grafts. Each has its own specific advantages and disadvantages. The aim of this review is to present the advantages and disadvantages of grafts used in the surgical treatment of Peyronie’s disease in the light of the current literature.
